Adult & Senior Outpatient Program

​Transitional Programs For Adults & Seniors 

This outpatient program provides care to adults and seniors with a mental health disorder including, but not limited to, bipolar disorder, schizophrenia, depression, anxiety disorder. The program offers group and individual therapy and is designed to enhance the physical, mental, cognitive, and social well-being of adults and seniors.

Transitional Care Program Features:​

  • Monthly one-on-one consultation with psychiatrist
  • Individualized treatment planning
  • Group therapy
    • Four therapeutic groups per day from 9 am – 2 pm with continental breakfast and lunch provided, a minimum of three days per week
  • Case management
  • Transition planning and aftercare
  • Life skills education
  • Individual therapy

    Before Selecting Your Appointment,

    there are some circumstances that require our assistance with scheduling.

    Please verify the following before proceeding:
    • I am age 35 or older.
    • I am NOT experiencing any issues, such as a lump, nipple discharge or other changes.
    • I was not diagnosed with breast cancer within the last three years.
    • I have not breast fed within the last six months.
    • It has been one year or more since my last appointment.
