Managing Your Diabetes

Managing diabetes is a comprehensive team effort. We will work closely with your primary care physician and referring specialists to develop a personalized care plan that will help you manage your diabetes better. ​

Whether you are newly diagnosed with diabetes or have been dealing with it for years, the registered dieticians and registered nurses at SSM ​Health St. Mary's Hospital can help you learn new ways to manage your disease.

Self​​​​​​-Management Program

St. Mary’s Diabetes Self-Management Program* is for people with type 1, type 2 or gestational diabetes. Information is provided in a group or individual setting on topics recommended by the American Diabetes Association. Medicare and most insurance plans cover part or all of the cost of the program with a medical provider order. For more information, call 573-681-3163 for Jefferson City or 573-582-6091 for Audrain. To schedule an appointment to attend, contact your medical provider. 

*The Diabetes Self-Management Education Program has been Recognized by the American Diabetes Association for Quality Self-Management Education ​

Prediabetes Education ​

St. Mary’s offers a prediabetes program to help people learn more about prediabetes and how to minimize risk for progressing to type 2 diabetes. The program is provided in a group setting. No medical provider order is required. The cost is $20 payable by check or cash at the door on the day of the program.     Before Selecting Your Appointment,

    there are some circumstances that require our assistance with scheduling.

    Please verify the following before proceeding:
    • I am age 35 or older.
    • I am NOT experiencing any issues, such as a lump, nipple discharge or other changes.
    • I was not diagnosed with breast cancer within the last three years.
    • I have not breast fed within the last six months.
    • It has been one year or more since my last appointment.
